Why this happens

Most chronic care falls short because a person's life and goals aren't in the plan

Why this happens

  • Misaligned plans — Up to 80% abandon care plans.
Today: Plans aren't built around lifestyle, culture, or goals.
  • Health literacy gap88% adults; 60% Medicaid are basic/below-basic.
Today: Trade-offs and risks are hard to parse; forms and terms overwhelm.
  • Confused care costs — about $238B wasted yearly.
Today: Confusion → skipped meds, avoidable ER visits, repeat calls.

In short, chronic care is prescribed, not lived.

Our vision and why we started

We're building a companion for people with chronic illnesses that aligns care to how they live and what they care about

Our vision

We're building a companion for people with chronic illnesses. It aligns care to how they live and what they care about — not just lab results. It listens to stories, understands goals, and works with doctors to shape meaningful, personalized care plans.

Why we Started
Mom's fall: I became the coordinator
I had no training. There were reports everywhere, too many different doctors. My Insight: Have clarity with one shared story.
Grandpa's Early diabetes: reading life with labs
Meals, sleep, and glucose trends all showed change early. Insight: Make sure to track story + signals together.
Meds vs mornings: Mom's trade-off
Her pain dropped but her mornings crashed. Insight: Make sure trade-offs are visible and can be adjusted.
The Caregiver load: sleep, stress, slips
Caring for Dad broke tons of routines. Insight: Include the care circle.

Credibility

We follow proven methodologies and align with value-based healthcare principles

What experts say
“Our ultimate goal, after all, is not good death but a good life to the very end.”

Dr. Atul Gawande highlights how simple systems and checklists help teams take the right step at the right time. We follow the same idea: a shared story, small guided steps, and feedback loops.
